Our Process
Step 1
Complete the enrollment form by uploading your child’s diagnostic report and insurance information. If you have questions you can leave them in the message portion of the form.
Step 2
Our Site Manager will contact you within 24 business hours to review your insurance benefits and to schedule an assessment. For SLP and OT services, additional items will be needed.
Step 3
Our Site Manager will email you an enrollment packet and schedule an assessment for services that you have selected.
